Note 1
One of the applications received in 21/22 did not meet the threshold, however a review was still undertaken as there were various agencies involved and to provide some reassurance to the person who applied for the review.
Note 2
All reviews will have an action plan produced which is sent to all agencies involved and also to the person who called for the review. Sometimes the only recommendation will be to identify the lead person and agency for updating and to confirm how they wish to be updated i.e. phone, letter, email and the frequency of contact.
